Job summary

BAYADA Home Health Care is seeking a Home Health Physical Therapist for a full-time opportunity to perform in-home visits in the Baltimore, MD area. The PT will assess mobility, develop care plans, and educate clients and families while documenting care using an EMR on a tablet.

The role requires an MD-licensed Physical Therapist, independence, and strong interpersonal skills to provide rehabilitative care to adults and geriatric clients in a multi-disciplinary team.

Qualifications

• Current MD-licensed Physical Therapist (PT).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, strength, balance, safety).
• Develop and implement individualized care plans with therapeutic exercises and gait training.
• Reassess and revise the PT care plan in collaboration with nursing.
