Job summary

BAYADA Home Health Care is seeking a Home Health Physical Therapist to perform skilled visits for adult and geriatric clients in the Baltimore, MD area. This full-time opportunity emphasizes high-quality, 1:1 rehabilitative care in clients' homes with a focus on independence and safety.

Qualifications

• Must hold MD Physical Therapist - PT license.
• Graduation from an American Physical Therapy Association or equivalent accredited program as indicated by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements on mobility, gait, balance, and endurance.
• Develop and implement individualized care plans including manual therapy and gait training.
• Review and revise care plans in collaboration with nursing and the multidisciplinary team.
• Educate clients and families on rehabilitative care and safety for independent living.
• Document observations and evaluations using EMR and required records.
