Job summary

BAYADA Home Health Care, Inc. is seeking a Home Health Physical Therapist to provide skilled PT visits in the White Marsh area of Maryland. This full-time role involves performing home health visits for adult and geriatric clients from our Baltimore office.

Duties include evaluations, plan development, and collaboration with a multidisciplinary team. The ideal candidate will hold an active Maryland PT license, demonstrate strong interpersonal skills, and be comfortable using EMR systems.

Qualifications

• Current Maryland Physical Therapist license (PT).
• Graduation from an ACPT/ CAPTE-accredited PT program with transcripts or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to patients in designated geographic territories.
• Assess mobility, strength, balance, and safety.
• Develop and implement individualized PT care plans, including exercises and gait training.
• Continually assess and revise the PT care plan with the multidisciplinary team.
• Educate patients and families in rehabilitative care and activities for safe home living.
• Accurately document observations, interventions, and evaluations in the EMR.
