Job summary

Qualifications

• Currently licensed MD Physical Therapist (PT).
• Graduate from an AMA/ACOTE approved PT program with transcript/diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated territories.
• Perform mobility and functional assessments.
• Develop and implement individualized home care plans with exercises.
• Regularly revise care plans in collaboration with nursing.
• Educate clients and families on rehabilitative care and safety.
