Job summary

Well Care Home Health of the Lowcountry, Inc. seeks a licensed Physical Therapist to assess, plan, implement, and evaluate patient care in a home health setting.

You will adapt plans to age and diagnosis, admit patients when PT is primary skilled need, and document per agency policies. Responsibilities include program development, efficient operations, flexible scheduling, and upholding mission and values while ensuring patient satisfaction.

Qualifications

• Graduate of Physical Therapy program from an accredited university.
• Licensure in SC or NC; CPR certification.
• Minimum 1 year of PT experience.
• Knowledge of home health PT procedures and modalities.
• Ability to transfer/guard patients and use assistive devices.
• Strong professional communication with patients, families, and staff.
• Physical ability for lift requirements and transfers.
• Clinically apply PT principles with critical thinking.
• Ability to see and communicate clearly for clinical tasks.
• Working knowledge of agency policies and documentation.

Responsibilities

• Assess, interpret, plan, implement, and evaluate patients per age/diagnosis.
• Admit patients to the agency when PT is a primary skilled need.
• Assist with program development and discipline-specific operations.
• Document and communicate information per guidelines and policies.
• Contribute to program effectiveness and efficiency.
• Organize and perform work effectively and efficiently.
• Maintain and adjust schedules to improve agency performance.
• Demonstrate daily alignment with agency values.
• Foster positive interpersonal relations with staff, patients, and families.
• Promote customer satisfaction and uphold agency mission daily.
