Job summary

Well Care Health seeks a compassionate Home Health Physical Therapist to join our team. You will provide physical therapy services in patients' homes, educate families, and collaborate with a supportive care team to ensure high-quality outcomes.

Requirements include graduation from an accredited PT program, NC/SC licensure (CPR preferred), and at least 1 year of experience in home health. The role involves lifting and patient transfers and offers growth opportunities within a values-driven

Qualifications

• Education: PT degree from an accredited university.
• Licensure in NC/SC plus CPR certification.
• Minimum 1 year of experience in PT/Home health.
• Knowledge of home health PT procedures and patient handling (modalities, assistive devices).
• Strong interpersonal and professional communication.
• Physical requirements include lifting >50lbs and transferring patients.

Responsibilities

• Provide PT care to patients in their homes.
• Develop and adjust patient care plans with the care team.
• Educate patients and families on health management and self-care.
• Document and communicate patient status per policies.
• Coordinate with team for program effectiveness and scheduling.
