Well Care Health is seeking a compassionate Home Health Physical Therapist to deliver high-quality patient care in patients' homes. The role focuses on assessment, treatment planning, and patient education within a supportive, mission-driven team.

Candidates must be a Graduate of a Physical Therapy program with licensure in NC or SC and at least 1 year of experience. The position includes a 40-hour work week with variable hours; driver’s license and reliable vehicle are required.

Qualifications

• Graduate of Physical Therapy program from an accredited university.
• Licensure in the state where providing therapy (NC/SC); CPR certification.
• Minimum 1 year of experience.
• Ability to transfer and guard patients and use therapy modalities.

Responsabilités

• Assess and plan patient-centered home health physical therapy.
• Educate patients and families on health management and self-care.
• Collaborate with care team to develop and adjust care plans.
• Document patient progress per agency policies.
• Demonstrate professional and compassionate patient care in the home setting.
