Qualifications

• Graduate of Physical Therapy program from an accredited university.
• Licensure in North Carolina or South Carolina; CPR certification.
• Minimum 1 year of experience in a related setting.

Responsibilities

• Assesses, interprets, plans, implements, and evaluates patients per age/diagnosis.
• Admits patients to the agency when PT is a primary skilled need.
• Communicates and documents according to agency policies and guidelines.
• Maintains schedule to enhance agency performance.
• Demonstrates commitment to agency values and mission.
• Ensures customer satisfaction and professional interactions.
• Performs duties in a safe and efficient manner daily.
• Adheres to safety and HIPAA/privacy requirements.
• Works shifts Monday–Friday, with flexibility for weekends/holidays as needed.
• Serves adolescents, adults, geriatrics, and pediatrics as applicable.
• Possesses valid driver’s license and reliable vehicle.
