Job summary

Well Care Health is seeking a compassionate Home Health Physical Therapist Assistant to provide high-quality care in patients' homes under the supervision of a licensed physical therapist. The role requires an Associate degree from an accredited PTA program and SC/NC licensure with CPR certification.

Expect 40-hour weeks and collaboration with the care team to educate patients and families, administer PT treatments, and develop personalized plans.

Qualifications

• Education: Associate degree from an accredited PTA program.
• Licensure in NC or SC; CPR certification.
• Minimum 6 months of PTA experience.
• Knowledge of physical therapy procedures and related equipment.
• Strong communication and teamwork skills.

Responsibilities

• Administer prescribed physical therapy treatments to patients in home care.
• Assist with developing and following individualized care plans.
• Educate patients and families on health management and self-care.
• Collaborate with the healthcare team to ensure high-quality care.
• Document patient progress per agency policies.
• Manage scheduling to optimize agency performance.
