Job summary

BAYADA Home Health Care, Inc. is seeking a Home Health Physical Therapist to perform patient visits in Waretown, NJ. The role requires a NJ PT license and at least 1 year of experience, with a full-time schedule and a $60–$72 pay-per-visit rate.

Qualifications

• Active New Jersey Physical Therapist license.
• Minimum 1 year of clinical PT experience.
• Ability to work independently and travel for home visits.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Conduct mobility, gait, balance, and strength assessments.
• Develop and implement individualized PT care plans including exercises and gait training.
• Educate clients and families on exercises, safety, and home programs.
• Document observations and interventions in the EMR system.
• Collaborate with nursing and rehab team to coordinate care.
