Job summary
BAYADA Home Health Care is seeking a licensed Physical Therapist for full-time home health visits in Cumberland County, NJ, based in Millville. You will join a multidisciplinary team to deliver rehabilitative care and skilled nursing to clients at home, performing home visits across designated territories.
The role requires an NJ PT license and at least one year of PT experience, with EMR proficiency and strong interpersonal skills.
Qualifications
- NJ Physical Therapist - PT license is required.
- Minimum of one year of PT work experience.
- Graduation from an APTA-approved PT program verified by transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational and problem-solving skills.
- Experience with EMR preferred.
Responsibilities
- Make home visits to clients in designated geographic territories.
- Perform diagnostic tests and measurements related to mobility, balance and gait.
- Implement wound care interventions and dressings.
- Develop and implement individualized PT care plans including therapeutic exercises and gait training.
- Collaborate with nursing to revise care plans as needed.
- Educate clients and families on rehabilitative care and safety at home.
