Job summary

BAYADA Home Health Care, Inc. in Linwood, New Jersey seeks a Home Health Physical Therapist for ongoing home visits to adult and geriatric clients.

The role supports full-time, part-time, or per diem opportunities with a $60+ per visit rate and a $10,000 sign-on bonus for eligible full-time staff. Responsibilities include evaluating mobility, developing individualized care plans, and collaborating with a multidisciplinary team.

Qualifications

• A current New Jersey Physical Therapist license.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the AMA.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements as described in the job posting.
• Develop and implement individualized care plans including manual therapy and gait training.
• Continually assess and revise the care plan in collaboration with nursing.
• Educate clients and families on rehabilitative care for health and independence.
• Accurately document observations and interventions using a tablet.
