Job summary

BAYADA Home Health Care, Inc. is seeking a Home Health Physical Therapist for Bayonne, New Jersey. This role focuses on providing skilled PT visits to adult and geriatric clients in Hudson County on a per-visit basis.

Ideal candidates have NJ PT licensure, clinical experience in home health, and strong documentation and EMR skills. This is an on-site opportunity with a sign-on bonus and comprehensive BAYADA benefits.

Qualifications

• NJ Physical Therapist license required.
• Graduation from a program approved by APTA or CAAHEA per transcripts/diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, joints, ambulation, strength, balance).
• Develop and implement individualized care plans with manual therapy, gait training, balance and other interventions.
• Continually assess and revise PT care plan with nursing collaboration.
