Qualifications

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform mobility assessments and functional tests.
• Develop and implement individualized care plans.
• Revise care plans with nursing and multidisciplinary team.
• Educate clients and families on rehabilitative care.
• Document observations and evaluations in EMR.
