Job summary

BAYADA Home Health Care, Inc. is seeking an experienced Home Health Physical Therapist to provide skilled visits to adult and geriatric clients in Atlantic County, New Jersey. The role supports full-time, part-time, or per diem schedules with a $10,000 sign-on bonus.

Qualifications

• Current New Jersey Physical Therapist license.
• Experience with home health or outpatient settings preferred.
• Ability to work independently and manage time effectively.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Assess and measure mobility, balance, and functional status.
• Develop and update individualized care plans with PT interventions.
• Document observations and care in EMR with accuracy.
• Educate clients and families to promote safety and independence.
