Job summary

Adara Home Health is seeking a licensed Physical Therapist to plan and provide physical therapy services in clients' homes across the St. Cloud area, including Little Falls and nearby cities.

Expect 1:1 client care with a flexible full- or part-time schedule and travel between homes. You will evaluate, treat, educate families, supervise PTAs/HHAs, and document progress while upholding confidentiality and safety.

Qualifications

• Minnesota Physical Therapy licensure with no restrictions.
• Minimum 2 years in health care; home care preferred.
• CPR certification recommended.
• Valid driver's license and access to vehicle for home visits.
• U.S. citizenship or valid work authorization.

Responsibilities

• Administers PT evaluations and develops Plan of Care.
• Provides PT treatments and assessments under physician orders.
• Supervises PTAs and HHAs; offers in-service training.
• Educates families and clients on home exercise programs.
• Maintains records, reports progress, and ensures confidentiality.
• Ensures safe home environments and reports abuse or neglect.
