Job summary

Adara Home Health, Inc. is seeking a licensed Physical Therapist to provide in-home therapy in the Saint Cloud area. You will evaluate clients, implement the Plan of Care, and deliver PT services in clients' homes with 1:1 visits, reporting to the Clinical Supervisor.

The role requires a MN license, at least two years of healthcare experience, strong communication skills, and a valid driver’s license. Base compensation includes a comprehensive benefits package and potential bonuses.

Qualifications

• Graduate of an accredited Physical Therapy program; MN license required.
• Minimum of 2 years healthcare experience; home care preferred.
• Strong interpersonal communication and teaching skills.
• Excellent written and oral communication.
• Valid driver’s license and proof of car insurance for business travel.
• Background checks and screenings required.

Responsibilities

• Administers physical therapy evaluations and develops the Plan of Care.
• Provides PT treatments, procedures, evaluations and diagnostic tests as prescribed by the physician.
• Translates exercises into functional activities and activities of daily living.
• Instructs on use and care of assistive devices.
• Develops plans for modifying equipment and home environments.
• Prepares clinical and progress notes and reports patient response to treatment to physician/case manager.
• Instructs and supervises PTAs and HHAs as appropriate.
• Provides family education and home exercise programs.
• Maintains confidentiality and safe client environment; reports concerns as needed.
• Performs other duties as assigned.
