Adara Home Health, Inc. is seeking a licensed Physical Therapist to provide in-home physical therapy services as part of a 1:1 client care team. The PT will assess, plan, and implement treatment programs in clients’ homes under physician orders and company policy.

Travel to client homes is required, with visits typically lasting 1–3 hours. A valid Minnesota license, strong communication, and independence are essential; the role includes potential bonuses and benefits under full-time eligibility.

Formación

• Graduate of an accredited Physical Therapy program; licensed as a Physical Therapist in Minnesota.

Responsabilidades

• Administers physical therapy evaluations and develops the Plan of Care.
• Provides PT treatments, evaluations, and diagnostic tests under physician direction.
• Trains and supervises PTAs and HHAs; contributes to staff education and case conferences.
• Maintains client records and documents response to treatment; ensures safety and confidentiality.
