Job summary

Adara Home Health in Minnesota is seeking a licensed Physical Therapist to provide in-home therapy under a physician-approved plan of care, travel to client homes, and collaborate with the Clinical Supervisor. The role offers flexible scheduling as either full-time or part-time, with mileage reimbursement and a competitive base salary.

The position covers the St. Cloud area and surrounding cities, with responsibilities including evaluations, treatments, and family instruction to support progress

Qualifications

• Must be licensed as a Physical Therapist in Minnesota.
• Minimum 2 years of healthcare experience; home care preferred.
• Strong communication, teaching, and organizational skills.
• Valid driver’s license and access to reliable transportation to multiple client sites.

Responsibilities

• Administers physical therapy evaluations and participates in Plan of Care development.
• Provides PT services in clients’ homes with physician-approved plans and translates exercises into daily activities.
• Prepares and maintains progress notes; communicates with physicians and case managers.
• Instructs and supervises PTAs and HHAs as appropriate; provides staff education.
• Supports families with home exercise programs and resources to improve client outcomes.
