Job summary

BAYADA Home Health Care, Inc. is seeking an experienced Physical Therapist to provide home health visits in Howard County, Maryland. The role is part-time or per diem, with hours 8:30a-5p, Monday through Friday, serving adult and geriatric clients in designated territories.

Qualifications include Maryland PT license and ability to work independently. EMR experience and strong communication skills are required to deliver high-quality care and documentation.

Qualifications

• Maryland Physical Therapist license required.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements for mobility, strength, balance, and safety.
• Implement wound care interventions using evidence-based dressings and topical agents.
• Develop and implement individualized care plans including manual therapy, gait training, and balance activities.
• Continuously assess and revise the PT care plan with nursing and multidisciplinary team.
• Educate clients and families on rehabilitative care and activities to promote health and independence.
• Document observations and interventions accurately using a touch pad tablet.
