Job summary
BAYADA Home Health Care in New Jersey is seeking a Physical Therapist (PT) for full-time home health visits in Atlantic County. New grads welcome. The role offers a sign-on bonus and the chance to build one-on-one patient relationships in clients’ homes.
You will perform comprehensive PT evaluations, develop individualized plans, and educate patients and caregivers while collaborating with a clinical team. Base pay aligns with experience and location.
Qualifications
- Must hold an active NJ PT license.
- Minimum 1 year of recent PT experience.
- Graduation from an APTA/CAAHEA-accredited program.
Responsibilities
- Perform home visits in designated Atlantic County territories.
- Complete comprehensive PT evaluations.
- Assess mobility, strength, balance, gait, transfers, endurance and safety risks.
- Develop individualized treatment plans to improve function.
- Provide therapeutic exercise, gait and balance training.
- Collaborate with nurses and other clinicians as part of a multidisciplinary team.
- Educate patients and caregivers on exercises, safety and disease management.
- Document patient care in the EMR system.
