Zusammenfassung

BAYADA Home Health Care, Inc. is seeking an experienced Home Health Physical Therapist to perform outpatient visits in Northfield, New Jersey.

The role supports adult and geriatric clients in the Atlantic County area, with a sign-on bonus and flexible full-time or part-time opportunities. The PT will collaborate within a multidisciplinary team, assess patient mobility, design individualized care plans, and document progress using EMR technology, enabling patients to receive hospital-level care

Qualifikationen

Current New Jersey Physical Therapist license.
Graduation from an APTA/CAAHEA-approved PT program.
Ability to work independently and manage time effectively.
Strong interpersonal, organizational, and problem solving skills.
Solid computer skills; EMR experience preferred.

Aufgaben

Make outpatient visits to clients in designated geographic territories.
Perform diagnostic tests and measurements (mobility, strength, balance, safety).
Develop and implement individualized care plans (therapeutic exercises, gait training).
Continuously assess and revise the PT care plan with nursing in a multidisciplinary team.
Educate clients and families on rehabilitative care for home health.
Document observations and interventions using a tablet and EMR.
