The role offers 40 hours/week, 8-hour shifts, staff employment, a $10,000 first-year bonus, and a pay-per-visit model with generous paid time off and benefits.

Qualifications

• Bachelor’s or Master’s in Physical Therapy from an accredited college.
• Licensed Physical Therapist in the state you will work.
• Minimum of one year healthcare experience.
• Current driver’s license and ability to travel to patient locations (~20%).
• Ability to meet charting deadlines and manage time.
• Experience with tablets, mobile devices and EMR software.

Responsibilities

• Deliver and manage patient care plans in home health.
• Collaborate with the care team to coordinate patient services.
• Maintain EMR documentation and ensure timely charting.
