Job summary

The Graham Healthcare Group is hiring a Home Health Physical Therapist to support patients in Mio, Michigan. This full-time position offers 40 hours per week, 8-hour shifts, and an annual pay range of $80K-$120K.

You will join Residential Home Health and Hospice within Graham's network, delivering patient care plans and collaborating with a supportive care team. A first-year $10,000 bonus is available and a competitive benefits package.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited college.
• Licensed Physical Therapist issued by the state in which you will be working.
• Minimum of one year healthcare experience.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
• The skills needed to self-manage your time and schedule.
• Demonstrated experience with tablets, mobile phones and EMR software.

• Manage and deliver patient care plans.
• Collaborate with a home health care team to implement care plans.
• Support flexible scheduling and charting to meet deadlines.
• Travel to patient locations as part of duties.
