Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited college.
• Licensed Physical Therapist issued by the state of practice.
• Minimum of one year healthcare experience.
• Ability to drive ~20% of the day to patient locations.

Responsibilities

• Provide management and delivery of patient care plans.
• Collaborate with a multi-disciplinary care team to coordinate services.
• Demonstrate strong charting and scheduling discipline.
• Utilize EMR software to document care and outcomes.
