Graham Healthcare Group is seeking a Home Health Physical Therapist in Hamtramck, Michigan to join our patient-centered care team. This full-time staff position requires a 40-hour workweek with an 8-hour shift and includes driving to patient locations.

Eligible for a $10,000 first-year success bonus and supported by a strong, team-based culture. The role emphasizes delivering and managing patient care plans using EMR systems and modern technology.

Qualifications

• Bachelor’s or Master’s in Physical Therapy from an accredited college.
• Licensed Physical Therapist issued by the state where you will work.
• Minimum of one year healthcare experience.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
• Strong charting and time management skills.
• Experience with tablets, mobile devices and EMR software.

Responsibilities

• Deliver and manage patient care plans in home health.
• Collaborate with the care team to coordinate services.
• Maintain accurate EMR documentation.
