Job summary

Graham Healthcare Group is seeking a Home Health Physical Therapist in Lapeer, Michigan to provide in-home patient care with 40 hours/week and an 8-hour shift. The role includes driving to patient locations, managing care plans, and leveraging EMR tools in a team-based environment.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy required
• Licensed Physical Therapist in the working state
• Must be willing to drive ~20% of the day and maintain charting deadlines
• Experience with tablets, mobile phones and EMR software preferred

Responsibilities

• Provide management and delivery of patient care plans in home health settings
• Collaborate with care team to ensure quality outcomes
• Maintain scheduling flexibility to meet patient needs
