Job summary

UnitedHealth Group is seeking a Physical Therapist - HAC to restore function, improve mobility and enhance quality of life for patients in a home-based care model. You will perform comprehensive assessments, develop plans of care with physicians, provide skilled PT interventions, and coordinate with interdisciplinary teams while maintaining thorough documentation and ensuring regulatory compliance.

Qualifications

• Graduate of an accredited physical therapy program (CAPTE accredited).
• 1+ year of experience in physical therapy; OR less than one (1) year with participation in an approved residency or mentorship program.
• 1+ year of experience in clinical assessment, documentation, and care coordination.
• Current, unencumbered license to practice physical therapy in the assigned state.
• Current CPR certification.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized physical therapy plans of care in collaboration with physicians.
• Provide skilled physical therapy interventions and modify treatment plans based on patient progress and clinical needs.
• Perform ongoing reassessments, including weekly evaluations, to support care planning and physician coordination.
• Collaborate with interdisciplinary teams to coordinate care and communicate patient progress and condition changes.
• Maintain accurate, timely documentation in compliance with clinical, regulatory, and documentation standards.
