Job summary

UnitedHealth Group is seeking a licensed Physical Therapist to provide skilled services in home-based care under physician-directed plans. The HAC role aims to restore function, improve mobility, and enhance quality of life for patients in their homes.

Qualifications

• Current, unencumbered license to practice physical therapy in the assigned state.
• Graduate of an CAPTE-accredited physical therapy program.
• Current CPR certification.
• 1+ year of experience in physical therapy; OR less than one (1) year with participation in an approved residency or mentorship program.
• 1+ year of experience in clinical assessment, documentation, and care coordination.
• 1+ year of experience in home health, hospital-at-home, or community-based therapy settings.
• 1+ year of experience working within interdisciplinary healthcare teams.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized physical therapy plans of care in collaboration with physicians.
• Provide skilled physical therapy interventions and modify treatment plans based on patient progress and clinical needs.
• Perform ongoing reassessments, including weekly evaluations, to support care planning and physician coordination.
• Collaborate with interdisciplinary teams to coordinate care and communicate patient progress and condition changes.
• Maintain accurate, timely documentation in compliance with clinical, regulatory, and documentation standards.
