Job summary

UnitedHealth Group is seeking a Physical Therapist to provide skilled services to patients in their homes, promoting mobility, restoring function, and improving safety and independence. You will work with patients, caregivers, and an interdisciplinary team to support recovery and prevent hospitalizations in the home setting.

Requirements include CAPTE-accredited PT education, state licensure, valid driver’s license, CPR, and 1+ year of home health experience or mentoring if less experienced.

Qualifications

• Graduates from a CAPTE-accredited physical therapy program.
• Current license to practice physical therapy in the state of assignment.
• Driver’s license and reliable transportation.
• Current CPR certification.
• 1+ year of physical therapy experience in home health care preferred; mentoring/residency for less experienced candidates.

Responsibilities

• Provide skilled PT services to home health patients.
• Perform reexaminations every 30 days and discharge planning as needed.
• Collaborate with patients, caregivers, and an interdisciplinary care team to support recovery and safety.
