Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists in the development of the therapy plan of care.
• Provides services within the scope of practice per state laws, in accordance with the plan of care, and with the health care team.
• Observes and reports the patient's response to treatment and any changes in condition to the supervising nurse or physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient, family, and caregivers regarding the plan of care and progress toward goals.
• Visits patients and completes progress notes per policy; develops PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and frequency of care.
