Caretenders, part of LHC Group and the Optum family of health care organizations, is hiring a Physical Therapist for Home Health in the Massachusetts area. You will perform assessments of functional status, mobility, endurance, and pain to help shape an effective plan of care for patients in their homes.

You’ll implement evidence-based interventions, coordinate with physicians and the care team, document visits, and educate patients and families.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification (preferred) or ability to complete within 90 days of hire.

Responsabilités

• Evaluates the patient's functional status and physical therapy needs; assists the physician in developing the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and in coordination with the health care team.
• Observes and reports the patient's response to treatment and changes in condition to the supervising nurse/physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Educates patient and family/caregivers about the plan of care, limitations, and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes per policy.
• Completes all patient evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
