Job summary

Caretenders, part of the LHC Group and Optum family, seeks a Physical Therapist for Home Health in Massachusetts. You will assess patient needs, develop and implement a treatment plan, and monitor progress in the home setting. Excellent teamwork with nurses and physicians is expected.

Responsibilities include evaluating functional status, planning care, and delivering interventions to improve mobility and pain, while documenting outcomes and maintaining state compliance. Sign-on bonus available.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance or access to reliable transportation.
• Ability to function in diverse home settings regardless of age, race, creed, or disability.

Responsibilities

• Evaluates patient functional status and PT needs; assists in developing plan of care.
• Provides services within state scope and plan of care.
• Observes and reports patient response to treatment to supervisor/physician.
• Treats patients to relieve pain and restore function while following standards of practice.
• Informs patient and family/caregivers about plan of care and progress toward goals.
• Visits patients per Plan of Treatment; documents progress notes per policy.
• Completes PT evaluations and develops plan of care per state guidelines.
• Reports outcomes and anticipated frequency of care.
