Job summary

Caretenders, a part of LHC Group and allied with Optum, seeks a Physical Therapist in Home Health to assess functional status, endurance, and pain, and to develop and adjust plans of care for patients in their homes in Indiana.

You will implement treatments, document progress, collaborate with nurses and physicians, and ensure compliance with state guidelines while benefiting from a comprehensive rewards package and opportunities for growth.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and access to reliable transportation.
• Preferred CPR certification.

Responsibilities

• Evaluates patient’s functional status and therapy needs; assists physician with plan of care.
• Provides PT services within state scope of practice and plan of care.
• Observes and reports patient progress to supervising staff.
• Treats patients to relieve pain and restore function; maintains maximum functioning.
• Instructs patient and family about plan of care, goals, and limitations.
• Completes evaluations and documents progress per policy.
