Job summary

Guardian Home Health, part of the LHC Group and Optum family, is seeking a Physical Therapist to evaluate, plan, and implement home-based therapy. You will assess functional status, plan care, and deliver interventions to improve patient well-being in home settings.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient’s functional status and needs; assists in developing the therapy plan of care.
• Provides PT services within scope of practice and per plan of care; coordinates with health care team.
• Observes and reports patient response to treatment to supervising nurse/physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning per standards.
• Informs patient and family about plan of care, limitations, and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes per policy.
• Completes evaluations and develops PT plan within state guidelines.
• Reports outcomes and projected frequency of care.
