Job summary

Holy Cross Home Care and Hospice (Maryland) seeks a Home Health Physical Therapist to deliver skilled, one-on-one therapy in patients’ homes, assess needs, and create individualized plans for safe recovery. The role supports independence, flexibility, and meaningful 1:1 patient relationships.

You’ll work with a team that values growth, supports professional development, and offers a patient-centered care model in the Prince George County area.

Qualifications

• MD licensed Physical Therapist with active Maryland license.
• Graduate of an accredited physical therapy program.
• At least 1 year of clinical experience, home care preferred.
• Valid driver’s license and reliable transportation.
• Ability to work independently and manage time effectively.

Responsibilities

• Provide skilled, one-on-one physical therapy in patients' homes.
• Assess patient needs and develop individualized treatment plans.
• Deliver evidence-based interventions and educate patients and caregivers.
• Support practice at the top of your license with flexibility, autonomy and meaningful 1:1 patient relationships.

Home Health Physical Therapist (PT)

As a Home Health Physical Therapist, you’ll provide skilled, one‑on‑one therapy services in the comfort of patients’ homes, helping them regain strength, mobility and independence. You’ll assess patient needs, develop individualized treatment plans, deliver evidence‑based interventions and educate patients and caregivers to support safe recovery at home.

Whether you’re experienced in home health or transitioning from hospital, outpatient, acute care or skilled nursing settings, this role offers the opportunity to practice at the top of your license with greater flexibility, autonomy and meaningful 1:1 patient relationship.

Minimum Qualifications

• Current licensure as a Physical Therapist in the State of Maryland


• Graduate of an accredited physical therapy program.


• At least one year of clinical experience (home care experience preferred).


• Valid driver’s license and reliable transportation.


• Ability to work independently and manage time effectively.
