Job summary

Holy Cross Home Care and Hospice (Maryland) is seeking a Home Health Physical Therapist to provide skilled, one-on-one therapy in patients' homes across Prince George County. You will assess needs, develop individualized plans and deliver evidence-based interventions to support recovery.

The role offers flexibility, autonomy and meaningful patient relationships, with growth opportunities and Day 1 benefits including medical, dental and vision coverage and a sign-on bonus.

Qualifications

• Current licensure as a Physical Therapist in Maryland.
• Graduate of an accredited physical therapy program.
• At least one year of clinical experience (home care preferred).
• Valid driver’s license and reliable transportation.
• Ability to work independently and manage time effectively.

As a Home Health Physical Therapist, you’ll provide skilled, one‑on‑one therapy services in the comfort of patients’ homes, helping them regain strength, mobility and independence. Minimum Qualifications:

• Current licensure as a Physical Therapist in the State of Maryland

• Graduate of an accredited physical therapy program.
