Job summary

Holy Cross Home Care and Hospice, part of Trinity Health At Home, is seeking a Home Health Physical Therapist to deliver skilled 1:1 therapy in patients' homes in Prince George's County, MD. You will assess needs, build plans, and educate families to support safe recovery at home.

Qualifications

• Current Maryland Physical Therapist license is required.
• Graduate of an accredited physical therapy program.
• At least one year of clinical experience; home care preferred.
• Valid driver’s license and reliable transportation.
• Ability to work independently and manage time effectively.

Responsibilities

• Provide skilled, one-on-one physical therapy in patients' homes.
• Assess patient needs and develop individualized treatment plans.
• Deliver evidence-based interventions and educate patients and caregivers.

Minimum Qualifications:

• Current licensure as a Physical Therapist in the State of Maryland

• Graduate of an accredited physical therapy program.

• At least one year of clinical experience (home care experience preferred).

• Valid driver’s license and reliable transportation.

• Ability to work independently and manage time effectively.
