Job summary

Holy Cross Home Care and Hospice (Maryland) is seeking a Home Health Physical Therapist to deliver one‑on‑one therapy in patients’ homes across Prince George’s County. You will assess needs, create individualized plans, and guide recovery with evidence‑based interventions.

The role is focused on independent work, flexible scheduling, and meaningful 1:1 patient relationships, with opportunities for growth and professional leadership within Trinity Health At Home’s network.

Qualifications

• Current licensure as a Physical Therapist in the State of Maryland.
• Graduate of an accredited physical therapy program.
• At least one year of clinical experience (home care experience preferred).
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide skilled, one‑on‑one PT services in patients’ homes.
• Assess patient needs and develop individualized treatment plans.
• Deliver evidence‑based interventions and monitor progress.
• Educate patients and caregivers to support safe recovery at home.

Minimum Qualifications:

• Current licensure as a Physical Therapist in the State of Maryland
• Graduate of an accredited physical therapy program.
