Job summary

Trinity Health At Home is seeking a Home Health Medical Social Worker (MSW) to deliver compassionate, in-home social work services, assess psychosocial needs and support families navigating illness and recovery. You will advocate for patient-centered goals and participate in care planning with autonomy and flexibility.

Qualifications

• Master's Degree from a school of social work accredited by the CSWE.
• Two years of social work experience in health care setting.

• Pay Range $30.31-$48.50 per hour (based on years of experience, skillset, etc.)

Minimum Qualifications

• Master's Degree from a school of social work accredited by the Council on Social Work Education

• State of Maryland Social Work License Required.

• Two (2) years of social work experience in health care setting.

• Ability to provide service to acutely and chronically ill patients

• Ability to complete written, verbal, and observational tasks indigenous to the position.
