Qualifications

• A current MD Physical Therapist license.
• Graduation from an APTA-approved program or AMA CAAHEA-accredited school.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility, strength, balance, and safety.
• Develop and implement individualized care plans with therapeutic exercises and gait training.
• Assess and revise the care plan with nursing in the multidisciplinary team.
• Educate clients and families in rehabilitative care to promote health and independence.
• Document observations and interventions in an EMR/touchpad tablet.
