Job summary

Providence at Home with Compassus seeks a skilled Physical Therapist to deliver home-based care, guide treatment plans, and restore patients’ functional independence in their own environment.

You will assess, treat, and consult under a physician’s plan of care, supporting adolescents, adults, and geriatric patients while collaborating with an interdisciplinary team. This is a collective bargaining position with a weekend day-shift schedule.

Qualifications

• Bachelor’s degree in Physical Therapy OR Master’s degree OR Doctorate in Physical Therapy from an accredited program.
• Current, unencumbered PT license (state) with licensure within 30 days of hire.

Responsibilities

• Plans and administers medically prescribed PT treatment programs.
• Completes comprehensive physical assessments and identifies needs for referrals.
• Performs tests and measures (ROM, strength) and functional analyses.
• Instructs patients and families on home exercise programs.
• Supervises PT assistants and aides.
