Job summary

Providence at Home with Compassus in Portland, OR is seeking a licensed Physical Therapist to join our home health team. You will evaluate patients of all ages, plan and implement PT treatments, and collaborate with an interdisciplinary team to restore or maintain patient well-being.

The role emphasizes patient-centered care, infection control, guidance to patients and families, and adherence to regulatory standards.

Qualifications

• BS in Physical Therapy from an APTA-approved program.
• Current State License as a Physical Therapist.
• Valid Driver’s License.
• Current CPR, TB screen and Hepatitis consent/declination.

Responsibilities

• Provides care with infection control measures per OSHA and company policy.
• Plans, implements, and evaluates patient-focused PT treatment.
• Performs initial and ongoing clinical assessments including PT-specific measures.
• Prepares and revises Plan of Care in consultation with physician and care team.
• Communicates effectively with patients, families, and health-care team to coordinate services.
