Providence at Home with Compassus is seeking a compassionate Physical Therapist for part-time, day-shift, weekend coverage. You will deliver personalized home-based PT, assess and treat patients, and guide families in care plans under physician supervision.

Responsibilities include comprehensive PT assessments, treatment planning, and supervising PT aides, with focus on independent function and safe home environments. A state PT license and BLS are required.

Qualificações

• Required - Bachelor's degree Physical Therapy from an accredited program
• Preferred - 1 year of PT experience in home health care or rehabilitation

Responsabilidades

• Plans and administers medically prescribed physical therapy treatment programs to restore function and relieve pain.
• Completes comprehensive physical assessments and identifies referrals to other disciplines.
• Performs tests and measurements such as range-of-motion and manual muscle tests.
• Instructs patient and family in PT procedures for home continuation.
• Supervises licensed PT assistants and aides.
