Job summary

Providence at Home with Compassus is seeking a licensed Physical Therapist to join our home health team in Oregon. You will develop and evaluate patient care plans, perform OASIS assessments, and collaborate with an interdisciplinary group to restore or maintain patient well-being.

Requires a BS in Physical Therapy, current state license, CPR/TB/Hepatitis clearance, and at least two years of acute care or rehab experience. Valid driver's license and strong communication skills are essential.

Qualifications

• Must have a BS in Physical Therapy from an APTA-approved program.
• Current State PT license.
• Valid driver’s license.
• Current CPR, TB screen, and Hepatitis consent/declination.
• Two years’ PT experience in acute care or rehabilitation.
• Good interpersonal skills.
• Knowledge of durable medical equipment.

Responsibilities

• Provides care with infection control measures per OSHA.
• Participates in in-service programs and meets regulatory requirements.
• Accepts clinical assignments within education and competence.
• Monitors cases for compliance with third-party payors.
• Communicates effectively with patient/family, team, and physician to coordinate care.
• Conducts initial and ongoing assessments including OASIS when appropriate.
• Oversees team members when applicable to deliver patient care.
• Prepares and completes a Plan of Care with physician input.
• Documents progress notes and orders; updates clinical records.
• Revises the Plan of Care based on evaluation data.
• Plans and provides treatment for neuromuscular, neurological, and orthopedic disorders.
• Instructs patient/family on transfer techniques and equipment use.
• Uses therapy equipment appropriate to patient needs.
• Adheres to company policies and information security training.
