Job summary

Atrinity Home Health, a part of LHC Group within the Optum family, seeks a Physical Therapist Assistant to provide treatment and patient education in home care. You will follow the established care plan, educate patients and families, and coordinate with the supervising Physical Therapist and care team.

The role requires current licensure to assist in physical therapy, a valid driver's license, and weekend availability. Competitive pay and comprehensive benefits are included.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Valid driver's license and vehicle insurance or access to public transportation.
• Available to work a weekend rotation.
• Current CPR certification or ability to complete within 90 days of hire.
• Ability to perform physical tasks required for patient care, including lifting, transferring, and assisting with mobility.

Responsibilities

• Follow state laws governing provision of physical therapy in home care; follow the plan of care defined by the supervising PT; recommend changes to the plan of care to the supervising PT.
• Instruct and aid patients in active and passive exercise, muscle re-education, gait, functional activities, ADL, transfer, safety, and prosthetic training.
• Observe, record, and report patient responses to treatment; coordinate care with the healthcare team as appropriate.
• Instruct patient, family, caregiver, and other health-care team members in therapy within the scope of the physical therapy assistant.
• Function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
