Top Echelon Software, LLC is seeking a dedicated Physical Therapist to join our home health team in the Denver area. This full-time role focuses on delivering patient-centered care in the home setting, evaluating needs, and coordinating with physicians and the care team to improve mobility and comfort.

You will document treatment plans, educate patients and families, and ensure adherence to agency policies and payer requirements while meeting productivity standards.

Qualifications

• Master’s degree in physical therapy (CAPTE-accredited).
• Current state license to practice physical therapy.
• Valid CPR certification.
• Must be able to travel to all business locations.

Responsibilities

• Provides patient care and documents treatment per physician orders and policy.
• Performs patient evaluations per agency procedures and selects diagnostic materials.
• Informs physician of assessment results and recommended interventions.
• Develops and documents treatment plans with intervention strategies and priorities.
• Delivers organized patient treatment including education and counseling.
• Presents patient information to physicians and updates the team on status and plan.
• Delivers treatments aligned with patient needs and payer guidelines.
• Sets measurable goals in collaboration with patients and team.
• Meets productivity standards and uses work hours efficiently.
