KingRecruitingServices seeks a dedicated Physical Therapist to join our home health team in Denver. This full-time role focuses on delivering patient-centered care in patients' homes, evaluating mobility and pain, and developing practical treatment plans to improve function and quality of life.

The ideal candidate has a CAPTE-accredited Master’s in Physical Therapy, is licensed in the state, CPR certified, and able to travel to multiple locations.

Qualifications

• Provides patient care and documents treatment per physician orders and policy.
• Performs patient evaluations per agency policy and procedure.
• Informs the physician of assessment results and interventions before initiating care.
• Develops and documents treatment plans with strategies and priorities.
• Delivers organized patient treatment including education and counseling.
• Presents patient information to physicians and keeps team informed of status.
• Provides treatment per patient needs and payer parameters.
• Sets objective, measurable goals with team and patient.
