Job summary

6 Degrees Search is seeking a dedicated Physical Therapist for full-time home health work in the Denver area. You will provide patient-centered care in patients' homes, perform evaluations, and develop treatment plans aligned with physician orders and agency policies.

The role requires a CAPTE-accredited PT degree, an active license in the state, CPR certification, and a driver’s license capable of traveling to multiple locations. Home health experience is preferred.

Qualifications

• Master’s degree in physical therapy (CAPTE-accredited program).
• Current state license to practice physical therapy.
• Valid CPR certification.
• Must be a licensed driver able to travel to all locations.
• Minimum nine months experience as a PT.
• Home health experience preferred.

Responsibilities

• Provides patient care and documents treatment per physician orders.
• Performs patient evaluations and documents findings.
• Informs physicians of assessment results and recommended interventions.
• Develops and documents treatment plans with measurable goals.
• Educates and counsels patients and families.
• Coordinates with physicians and care team on patient status.
• Meets productivity standards and manages time efficiently.
