Job summary

Talent Solutions Partners seeks a dedicated Physical Therapist for a full-time home health role in the Denver area. You will provide patient-centered in-home care, perform evaluations, and implement treatment plans in collaboration with physicians and the care team.

Qualified candidates have at least nine months of PT experience, a CAPTE-accredited degree, and active licensure. CPR certification and a valid driver's license with travel readiness are required.

Qualifications

• Minimum nine months experience as a physical therapist.
• Preferred community/home health care experience.
• Must be licensed to practice as a physical therapist in the operating state.
• CPR certification required.

Responsibilities

• Provide patient care and document treatment per physician orders.
• Perform patient evaluations and select diagnostic materials.
• Inform physician of assessment results and recommended clinical interventions prior to initiating care.
• Develop and document appropriate treatment plans with priorities.
• Provide education and counseling to patients.
• Present pertinent patient information to physicians and keep team members informed of patient status and plan.
